Project Summary

NittanyNav is an application with augmented intelligence (AI) that aims to help patients with diabetes and their providers address social determinants of health (SDoH). Utilizing pre-established databases, NittanyNav will provide suggestions and assistance for patient concerns, such as food resources, based on zip code, patient needs, and other sociodemographic factors through a recommender system model. This will utilize AI to better understand our patient population and learn from their choices and feedback.

Our tool will augment existing health care provided through traditional measures we are familiar with. Market analysis has shown that a sizable segment of patients look up medical information online, but presently, no products or interventions can recommend patients to local resources based on their specific social and medical needs.

In summary, our objective is to help minimize health disparities in patients with diabetes by intervening on SDoH. In the future, our application can serve patients directly through an interactive, simplified and engaging interface to connect patients with community resources. Through this work, we hope to achieve better disease control and better health outcomes.
